The basic $30 rebate is available to anyone buying 8+ boxes and following the rebate instructions, the $60 rebate is only available for customers who are NEW Acuvue Oasys wearers. General Steps
- Purchase the required number of boxes of ACUVUE Brand Contact Lenses within 90 days of your eye exam. See Display above for specific requirements for this ACUVUE product.
- To receive a Rebate form visit www.acuvue.com/rebates and download the form.
- Fill out the rebate form and follow its instructions.
