Purchase the required number of boxes of ACUVUE Brand Contact Lenses within 90 days of your eye exam. See Display above for specific requirements for this ACUVUE product.
To receive a Rebate form visit www.acuvue.com/rebates and download the form.
Fill out the rebate form and follow its instructions.

Acuvue 2 - Love 'em
I have only been wearing Acuvue 2 lenses since last week and I love them. They are so comfortable that I forget I have them in! I recommend Acuvue 2 contacts!
